General Inquiry
A general inquiry is when you have a question or need more info about something. Like if you're curious about how something works or you're not sure about a certain topic, that's when you submit or make an inquiry. It's all about getting answers and understanding things better.
Request
A request is when you need something specific done or arranged. It's like asking for help or getting something you need. So, if you need to file a course completion request or request to shift programs, that's when you submit a request.
Feedback
Feedback is like sharing your thoughts or feelings about something. Whether it's good, bad, or just suggestions for improvement, your feedback is valuable. So, if you loved a mentor's synchronous class or you think there should be more course offerings, that's your chance to give feedback and help us serve you better.

Academic-related Services

Request
- Academic Advising - Select this academic-related service if you would like to schedule a consultation with your program head for special inquiries regarding your curriculum or if you're seeking guidance on how you can graduate at the soonest possible time. Your program head may assist you with concerns specific to you.
- Awards and Recognition - Select this academic-related inquiry if you would like to inquire about your eligibility for awards and recognition.
- Request for Credit Evaluation (for shiftees) - Students who shifted programs are advised to undergo this process to credit their previously taken courses to their new program.
- Request for Credit Evaluation (within the initial term) - Students who have prior college experience before enrolling at MMDC may request for their previously taken courses to be credited here at MMDC. Please note that this request is only possible within their initial term at MMDC. Thus, all the requirements such as Transcript of Records (for evaluation purposes), course description, and Request for Credit Evaluation Form (RCEF) must be submitted within the given timeframe to prevent it from lapsing.
- Course Completion - If you have incurred an INC or incomplete grade in the previous term, you need to submit a course completion request with complete requirements to the Integrated Advising Team AND submit your pending milestone/output submissions to your mentor or your program head. You are given only one term to complete your INC grade otherwise, it will be marked as failed. Select this academic-related service when you're ready to submit your course completion request or need assistance from your Integrated Advisor regarding this service.
- Request for Study Overload - Select this academic-related service if you would like to enroll beyond the maximum course load allowed (18 units). Please note that this service is only available to graduating students.
- Grade Concern/ Consultation - Select this academic-related service for inquiries, concerns, or requests for consultations regarding your grades.
