What are student document requests?
Student Document Requests, also referred to as Applications for Various Documents, is a service provided to students who need official copies of their school records. This includes, but is not limited to, transcripts, certificates, enrollment certifications, and other academic or administrative documents.

*Note: Processing of document requests will only commence once all valid documentary requirements are submitted, the processing fee payment is posted, and the physical copies of your Admission documents are received.
Request Process
To facilitate the process and provide you with the requested documents, please follow the
steps outlined below:
1. Complete the Request Form: Please take a moment to fill out the form, indicating the specific documents you wish to request.

Download this form if you wish to transfer out and would like to request your Transfer Credentials: Application for Transfer Credentials.
2. Processing Fee: For each document request, there will be a corresponding processing
fee. Please refer to the table of fees. If you are requesting multiple documents, please
ensure you calculate the total fees accurately. Only indicate any of the following processing
codes when making a payment: 2020654321, 2020123456, and 2020765432. You can

3. Submission: After completing the Application for Various Documents Form and completing the payment for the processing fees, please submit a document request Google form by clicking the button below, and upload the document request form and proof of payment.
IMPORTANT: Please submit one Google Form for EACH document request.
