Form 137 Request Letter
One of the major admission requirements for incoming freshmen is their Form 137 or School Form 10 (Learner's Permanent Academic Record) with the remarks: “Copy for Mapúa Malayan Colleges Laguna.”
Some schools require that an official letter be provided to facilitate the release of this document to their students.
Process
To request an F137 Request Letter from us, kindly submit a ticket through our Support Page or send us an email at iaservices@mmdc.mcl.edu.ph stating your request with the following information:
- Student Name / Name of Student
- School Name / Name of School
- School Address / Complete Address
You will be receiving a digital copy of your F137 Request Letter. Please allow 1-3 business days for document processing.
