This article explains how to request a Form 137 (School Form 10 / Learner’s Permanent Academic Record) request letter, which is often required by schools to release your academic records.

Who Needs This

Incoming freshmen may need a Form 137 request letter with the remark:

“Copy for Mapúa Malayan Colleges Laguna”

Some schools require an official letter to release this document.

How to Request a Form 137 Letter

1. Submit a Request

   • Go to the Help Center to submit a ticket.

2. Include the Following Information

   • Full Student Name

   • School Name

   • Complete School Address

3. Processing Time

   • A digital copy of the Form 137 request letter will be sent to you.

   • Allow 1–3 business days for processing.
