As part of the pre-practicum requirements, students must secure a certification to be in good physical and emotional/ psychological health before they are endorsed for internship. This is to ensure that all students are physically fit to take on real-world responsibilities in their Host Training Establishments (HTEs), and are emotionally prepared to navigate workplace dynamics.
This requirement aligns with CHED Memorandum Order No. 104, s. 2017, which mandates that student interns undergo both a physical and psychological examination conducted or certified by a DOH-accredited clinic or hospital.
Required Health Clearances
The following health assessments must be completed before enrollment in the Practicum course:
1. Physical Examination
- Student is encouraged to have this done by their preferred laboratory/ diagnostic center. The examination must be conducted or certified by a DOH-accredited clinic or hospital.
- This may also be completed through MMDC’s Annual Physical Exam (APE) program. This service is part of the student’s health benefit and comes at no additional cost. - Physical Evaluations are only valid for 6 months from the date of assessment.
2. Psychological Assessment
- Must also be conducted or certified by a qualified psychological service provider
- The cost of this assessment is to be shouldered by the student
- The student may choose a preferred provider that meets MMDC guidelines
- Psychological Evaluations are only valid for 6 months from the date of assessment.
Requesting a Referral Form
Students may request a referral form to present to a clinic or psychological center, if needed, for the purpose of obtaining the required physical or psychological evaluation..

The student may use and present the referral form at a preferred provider.
Submitting Results for Endorsement (Pre-Internship)
Once the exams are completed, the results must be submitted along with the đź”— Practicum Form 01: Practicum Intent Form as follows:

Please make sure that both CHSW and CGC sign on the same copy of your Practicum Intent Form.
To do this, submit your results one after the other—not at the same time—to ensure that each office can review and signify their endorsement (signature) on the same document. - If the results indicate medical or psychological conditions that require clarification, the student may submit additional documentation confirming fitness to engage in practicum.
- Physical and Psychological Evaluations are only valid for 6 months.
- In cases where there are observed mental health issues or challenges reflected in the results of the psychological evaluation of the student, a psychological interview with the CGC will be conducted virtually with the concerned student before issuing endorsement.
Reminders:
- All health documents must be submitted, clearance and endorsement obtained before the start of practicum.
- Only students with endorsed Practicum Intent Forms from both CHSW and CGC will be cleared for practicum placement.
- Upon completion of endorsement from CHSW and CGC, the student may proceed with the submission of their Practicum Intent Forms to their Program Heads or Practicum Coordinators.
Suggested Psychological Centers
The following are recommended psychological service providers compiled by MMCL’s Center for Guidance and Counseling (CGC). These clinics are known to offer psychological assessments suitable for internship clearance.
Note: This is not an official or exclusive list. Students may also choose to undergo their psychological assessment at any DOH-accredited clinic or hospital that offers psychological evaluation services.
In most cases, the clinics listed below do not require a referral letter, but students are encouraged to check with the provider directly for any requirements.
